Early Years Family Hub Practitioner Family Hub, Bolton 18/11/2024 5 months initially 4days per week Office based (flexible with what days and times around 8.30am 5pm 30 hrs)
- Deliver a range of preventative evidence-based interventions, on a one-to-one or group basis, and support services to families, this can include Home Learning Environment interventions and parenting support
- Use assessments and interventions to plan and evaluate care following clinical pathways
- Act as a key contact for all families
- Help families to understand the support that is available to them and share information to support new and expectant parents/carers during their transition to parenthood
- Supporting parent and child sessions to improve language development
- Observe, assess and enhance the development and behaviour of children, focusing on the principles of school readiness
- Promote positive healthy lifestyles and emotional health and wellbeing, providing relevant access to health care services
- Working in the community and liaising with schools and day care settings
- Work in partnership with other key stakeholders to safeguard children and young people
- Make referrals to Family Hub and Family Hub Network as appropriate
- Keep and maintain accurate records of work
- UK Driving Licence, access to a vehicle, business insurance
- Relevant level 3 qualification in Early Years, Education or Health and Social Care
- Must have school/early years/nursery experience
- Ability to support and observe a child s development stage
- Have a thorough knowledge of the Early Years Foundation Stage and how this relates to children s development and learning
- Enhanced Child and Adult DBS registered to the update service
