Role Description:

We are looking to recruit experienced support worker with relevant experience supporting individuals with several types of Dementia. Lime House provides residential support and care in a home from home environment for elderly people including eleven places for individuals with dementia.

You will have responsibilities, be enabled and supported to achieve great outcomes for our beneficiaries. You will be collaborating with an established team and receive support from our business functions.

Our philosophy at Lime House is to provide an excellent standard of care in pleasant surroundings. At Lime House we care for the whole person which includes meeting their physical, emotional, and spiritual needs. We respect dignity and encourage individuals to be as independent as possible.

We provide excellent care in homely, pleasant surroundings offering privacy and independence. There is a sensory garden to rear of the building to enable residents to relax and take time out in a tranquil setting.

Experience and Skills:

  • You must be enthusiastic about providing outstanding care to our clients.
  • Be willing to undertake training to achieve your level 3 Diploma in Adult Health & Social Care which is fully funded by Nugent.
  • 6-month experience working in a similar setting.
  • Experience in supporting individuals who have various forms of Dementia.
  • Experience in providing person centred care
  • A good understanding of providing Person Centred Care
  • The above position is subject to an enhanced DBS check.

he successful candidate will be required to work 37.5 hours per week, working Monday - Friday, 9AM - 5PM. Temporary cover is required for approximately 3 months. In this position, you will be expected to;- Hold a caseload of up to 14 key clients and conduct regular key working sessions resulting in agreed SMART Support Plans which are reviewed regularly- Complete comprehensive risk and needs assessments- Support all clients to participate in Education, Training, Employment and Volunteering opportunities as appropriate to the development of their skills, assets and aspirations- Deliver one-to one and group support opportunities within the accommodation and in the local community aimed at increasing resilience in the clients we work with- Be flexible, reflective and creative in your engagement with clients and personalised in your support to them- Support clients with move-on and help them progress towards independence- Involve clients in the decisions made about them and encourage participation in the organisations wider community programme of activities- Prepare the accommodation for new clients to move into and support clients to settle in- Send daily occupancy lists to the council- Be responsible for cleaning tasks and domesticated duties within the accommodation- Ensure the safety and wellbeing of clients in the service including using Safeguarding and emergency alert procedures as applicable- Be responsible for the health and safety responsibilities of the building, ensuring regular checks are completedTo apply for this role, you must have;- Experience of working with those experiencing homelessness and an understanding of the needs of people who have experienced homelessness, poor mental health, substance misuse- Experience of holding a caseload, conducting support plans, risk assessments and needs assessments- Effective collaborative working- Knowledge of Health and Safety and a clear understanding of Safeguarding requirements and procedures- High level understanding of professional boundaries and ability to maintain boundaries- The ability to use IT systems including Microsoft Office and databases and good literacy and numeracy skills
